When have you seen other leaders make this mistake? What happened?

I think this happens, especially, when leaders are under time pressure. They then focus on procedures and systems, and neglect relationships. That is usually when they jump from direction to achievement. The focus is on getting the results, but the people who need to do this might not be aligned at all.

When have you made this mistake yourself? What happened?

As mentioned above this has happened to me when I focussed more on the outcome than the process.
